Liver Disease Care Navigation

Living with liver disease—such as MASLD, MASH, NASH, or fatty liver disease—can be confusing and overwhelming, especially when trying to understand diagnoses, testing, and treatment options. Many patients struggle to know where to start, which specialists to see, or how to manage the many appointments and decisions involved in their care.
You Matter Patient Advocacy Group helps individuals and families navigate the liver disease journey with guidance, education, and support. We work alongside patients to help them understand their diagnosis, prepare for medical appointments, communicate with providers, and identify the next steps in their care.
Our advocacy services include care navigation, provider communication support, education about liver disease, and referrals to appropriate specialists—including Hepatologists and liver care teams. By helping connect patients with the right providers and resources, we aim to reduce confusion, improve access to care, and ensure individuals feel supported throughout their liver health journey.
Unique to us is that we maintain a database of Hepatologists (some might be Gastroenterologist/Hepatologists) who manage NASH/MASH/MASLD and patients experiences with providers. This allows for easier referrals. Its a patient provided database. We’re situated in Northeast Ohio, so it is Ohio focused for the time being. We also connect with other liver disease organizations for support, information and guidance, which is all apart of the care navigation process.
Our goal is simple: to make sure no one has to face liver disease alone while navigating the healthcare system.
