You Matter Care Navigation

You Matter Liver Care Navigation is a program designed to help individuals and families navigate the complex journey of MASLD, MASH, NASH, and fatty liver disease with guidance, education, and compassionate support. Receiving a liver disease diagnosis can be overwhelming, and many patients are left trying to understand medical terminology, testing, and treatment options while also figuring out where to turn for the right care. Our goal is to walk alongside patients so they do not have to navigate this process alone.

At You Matter Patient Advocacy Group, we work directly with patients to help them understand their diagnosis, prepare for medical appointments, communicate with healthcare providers, and identify the next steps in their care. Our advocacy services include care navigation, provider communication support, education about liver disease, and referrals to appropriate specialists—including hepatologists and liver care teams. By connecting patients with the right providers and resources, we aim to reduce confusion, improve access to care, and help individuals feel supported throughout their liver health journey.

One of the unique aspects of our program is that we maintain a patient-informed database of hepatologists and gastroenterologist–hepatologists who manage MASLD and MASH. This database includes shared patient experiences with providers, helping make referrals easier and more informed for those seeking specialized liver care. Because You Matter Patient Advocacy Group is based in Northeast Ohio, this resource is currently focused on providers within Ohio, with plans to expand as our network grows.

As part of the care navigation process, we also connect patients with other liver disease organizations, educational resources, and support networks at both the national and international level. These partnerships help ensure individuals have access to additional information, community support, and advocacy resources throughout their care journey.

Our goal is simple: to make sure no one has to face liver disease alone while navigating the healthcare system.
